Financial Hardship:

I Understand that you may have times of financial hardship. I have some options to help you through these difficult times.

I do not currently offer a sliding scale fee. I do however offer a temporary scholarship for reduced fees. The scholarship can last for 2 months with an option to extend based on circumstances.

I also offer 30 minute sessions to save on costs.

Insurance:

Currently I am able to accept only the following insurance plans:

• Regence Blue shield
• Out of Network provider coverage

Your insurance provider may be able to cover out of network provider costs. In these circumstances I will help you with the necessary billing information.

To determine if you can be reimbursed for services consider the following questions:

• Do I have mental health insurance benefits?
• Does my plan reimburse for out of network providers?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Hours

• Office hours by appointment only
• Special hours including evening hours may be available by special appointment.
• Telephone hours: 9am to 5pm

Payment

• Cash, Check, Visa, Mastercard, and American Express are accepted as payment.
• Payment is due at the time of services rendered.

Cancellation Ploicy

• I require 24 hours advanced notification if you need to cancel a session. For cancellations of less than 24 hours notice I charge $50
